Stroke and Neurological Rehabilitation

Post-stroke and neurological rehab programs are designed to help the patient attain their highest level of function and independence so they may return to their homes, family, community and employment.  Treatment programs are individualized to meet each patient’s needs and include rehabilitation specialists such as physical, occupational and speech therapists. 

Our team approach focuses on accomplishing the following:

  • Activities of daily living (ADLs) such as eating, dressing, bathing, toileting and housekeeping skills
  • Mobility skills such as walking, transferring, getting in/out of bed, or wheelchair propulsion
  • Neurological reeducation to improve balance and safety
  • Therapeutic exercise and activities to increase control and muscle balance in weak limbs and trunk
  • Bowel and bladder retraining
  • Communication skills
  • Reeducation in swallowing/eating
  • Improving cognitive skills
  • Education for patient and family regarding the disease and disease process
  • Goal setting with the patient and family
  • Discharge planning including Home Care, Outpatient and Home Equipment referrals

The Stroke and Neurological Rehabilitation program at HCPC offers a comprehensive range of services with a patient often receiving up to three hours of therapy a day, 7 days a week.  As a result, we can assist the patient transition from hospital to home at an optimal, functional level.
